httpauth: If REMOTE_USER is empty, behave as though it was unset
...
A frequently cut-and-pasted HTTP basic authentication configuration
for nginx sets it to the empty string when not authenticated, which
is not useful.

partially fix httpauth canedit hook
...
My logic was right before. Cleaned up some code.
(Page creation is still a problem.)
Also, I removed the Edit url munging, because that is not
necessary with the canedit hook, since canedit will handle
redirection through cgiauthurl if necessary.

httpauth: When cgiauthurl is configured, httpauth can now be used alongside other authentication methods (like openid or anonok). Rather than always redirect to the cgiauthurl for authentication, there is now a button on the login form to use it.

* pagespec_match() has changed to take named parameters, to better allow

for extended pagespecs. The old calling convention will still work for
back-compat for now.
* The calling convention for functions in the IkiWiki::PageSpec namespace
has changed so they are passed named parameters.
* Plugin interface version increased to 2.00 since I don't anticipate any
more interface changes before 2.0.
